Written by Jessica Franco

DataScope’s Form Builder allows you to create, edit, and customize forms quickly and easily, adapting them to any operational process.

1. How to get started

To create or edit forms:

  1. Log in with your username and password

2. Where can you create forms?

Once you are in the platform:

  • Go to the Forms tab

  • There, you can:

    • Create a form from scratch using the “Create” button

    • Edit sample forms included in your account

    • (Optional) Use templates or AI-powered automatic generation

3. How does the Form Builder work?

The builder was designed to be intuitive and flexible, allowing you to create forms in just a few steps.

Main elements:

  • Form name
Located in the upper-left corner. Defines the name of the form.

  • Add questions
Use the “+” button or “Add new question”

  • Question type
Choose from different types, such as text, selection, photo, and more.

  • Duplicate questions
Ideal for saving time when repeating structures.

  • Form structure
View and navigate through the form using the side menu.

  • Save changes
Click “Save form” to avoid losing your progress.

4. Question settings

Within each question, you can configure:

Basic fields

  • Title
The name of the question (visible in the app, PDFs, and exports).
⚠️ It cannot be repeated within the same form.

  • Instruction
Help text for the user (only visible in the app).

  • Sections
Allow you to group questions (visible in PDFs).

Quick actions

  • Duplicate question

  • Delete question

  • Add a new question below

5. Advanced options

Advanced options may vary depending on the question type you are using.

In other words, not all questions have the same settings. Some options may appear or change depending on how the question is configured.

For example:

  • Required → available for most questions

  • Skip questions → enables conditional logic

  • Comments, images, or issues → available for checklist-type questions

  • Related lists → available for questions using data lists

💡 This allows you to customize each question according to the information you need to collect.

6. Finish and test your form

When you are done:

  1. Click “Save”

  2. Test your form using:

📱 Mobile app (recommended)

  • Ideal for field use

  • Supports photos, signatures, GPS, and offline mode

💻 Web

  • You can also complete forms directly from the web platform

  • Simply go to Forms > Answer

💡 Important

  • Some features may vary between the app and web

    • Example: the scanner is not available on the web

  • You can also share the form using a public link so it can be completed from any browser

💡 Best practices

  • Use clear and unique names for each question

  • Add instructions when necessary

  • Group questions into sections to keep your form organized

  • Always test your form before sharing it
