This question type allows you to select fixed alternatives for each item of a custom list.
The easiest way to explain how this type of question works is to view it as multiple "multiple-choice" questions on the same screen with same and predetermined answers for the entire list.
This question can be used, for example, to evaluate different products under the same standards (such as control the quality of oranges, apples and mangoes is good, bad or regular) or to notify the activity that was carried out in different locations (such as reporting cleaning, organizing or both, in bathrooms, rooms or gardens).
Here's a demo of how to set up this question:
To add the question to your form and define its function:
1. Click the "+" button (blue color) or Add New Question in the lower-left corner.
2.In the new question, define:
Title: Define the question title. This field appears in the exported document.
Description: you can enter a description or explanation related to the question. This field appears only in the app, and does not appear in the exported document.
3. Adding questions:
Massively: In "Data List Source," click to select a list of questions previously loaded on the platform. To learn more about how to load your lists, to learn how to add lists, click here.
Manually: If you prefer to add questions manually, click on "New Data List Source" > "Add List Value." With each new click, a new field will be added to add your questions. If you wish to delete any of the questions, simply click on the "x" icon.
4. To add the answer alternatives (e.g., good, fair, poor): simply use the fields that the platform presents by default in the "Alternative Column." In this section, the user can choose only one alternative from the available ones.
5. Click "Add Option" and a new box will appear on the screen where you can add another answer alternative. On the other side, if you want to delete any, you must press the "x" button on the right side of the box.
6. To save the question, click the "Save" button.

📌 Setting up a Comment Question with Images or Text
Now it is possible to customize the configuration to require comments or images, whether in one or multiple selected options.
With this improvement, you can obtain more detailed information and additional justifications when elements do not meet the requirements or are not applicable, facilitating the analysis and tracking of review results.
Let's imagine that you are using a form to review safety elements. To evaluate each element, users can choose between three options: "Yes," "No," or "Not applicable.
By selecting the "Comment" option in the advanced settings of a checklist-type question, you can specify that if users choose "No" or "Not applicable," they will be prompted to add an explanatory comment and/or attach an image. You have the flexibility to choose between requiring both or just one of them, depending on your needs.
To activate the Comment and Image Question option, simply create a checklist-type question and enable the comment option. A modal of options will allow you to configure the requirement for the note, image, or both.
It is important to note that when configuring the form, it will not be possible to activate the requirement for images in questions with a list exceeding 50 elements.

In Web Forms, the option to attach images will be activated when an option is selected within the question. Keep in mind that if the question has a data list with more than 50 elements, it will not be possible to attach images.
It is possible to attach up to 10 images for each option. To delete an image, simply click on the trash can icon next to each image.
If you try to submit the form without attaching the required image for an option, a specific error will be generated for that item.

Here's an example of how the question would be viewed in the mobile app:

You can attach a maximum of 10 images, regardless of whether they are mandatory or not.
Only after selecting an option in an item, buttons to add notes or images will be displayed. You can upload photos from both the gallery and the camera, combining both if necessary.
If you try to upload more than 10 images from the gallery, a warning will be displayed indicating that only the first 10 selected images will be kept.
To indicate that a comment or image is required, the item will be highlighted with a red border until the required comment or image is added.
In lists with more than 50 elements, it will not be possible to add images.
To delete an image, simply click on it, view it in detail, and use the delete image button.
This type of question only supports one image quality, which is low and not configurable.
