An Issue is used to report a problem, failure, or deviation found in the field and assign the people who need to resolve it. You can create it from the web, from the app, or directly while answering a form with a checklist question.
📌 The Issues module is available on the Professional and Enterprise plans. Accounts on the Free or Starter plan can activate a 30-day trial.
📌 Where to create an Issue
From the web platform
Select Create + and then Create issue. You can also use the Create issue button at the top of the platform.
The Create issue panel opens. Fill in the fields.
Save the Issue.
From the mobile app
There are two ways to create an Issue:
In the Issues section, with the + New issue button.
From anywhere in the app, with the ⚠️ icon. This lets field workers report a problem on the spot, without leaving what they’re doing.
From a checklist question
While answering a form, on the web or in the app, you can create an Issue from a checklist question. You can create as many Issues as there are items in the question, so each problem is handled separately.
When an Issue is created from a checklist, it shows the source question and the option that was selected (for example: Bad, Fair, or Good). This makes the context clear without reviewing the whole form.
If you want certain checklist answers to require creating an Issue, see Mandatory Issues in checklist questions
📌 Issue fields
Fields marked with an asterisk (*) are required.
Required information
Issue type: the type that best describes the problem. By default, “Maintenance”, “Inspection”, and “Other” are available.
Title *: the Issue’s name, giving a quick reference to the problem. Describe it in a few words.
Description *: the context, possible causes, and any information that helps resolve it. If you include a URL, the link will be clickable on the web and in the app.
Evidence: you can attach up to 10 images, which appear in the Issue chat from the start. If the Issue is created from a checklist question with images, they’re included automatically and can’t be removed.
Asset: if you have the Assets module enabled, you can link a piece of equipment, vehicle, or facility to the Issue. By default, No asset is shown.
Assignee(s) *: the people in charge of closing the Issue. They’re the only ones who can edit it.
Due date: the deadline (date and time) to resolve the Issue.
Priority *: Low, Medium, High, or Critical. By default, Medium is selected.
Optional information
Location: where the problem was found or happened.
Invite participants: other users needed to resolve the Issue. They’re added when the Issue is created.
Custom fields
If the Issue type has custom fields, they appear when you create and edit the Issue. Required ones won’t let you save if they’re empty. See Custom fields by Issue type
📌 What changes when you choose an Issue type
When you select a type, the Issue takes on the configuration defined for that type:
Information configured in the type is filled in automatically and can’t be edited. Fields the type leaves empty can be filled in.
If the type has deadlines by priority, the due date is calculated automatically based on the priority you choose.
If the type has a default priority, you won’t be able to change it, but you’ll see its description.
The type can require evidence, a location, or an asset. In that case, you can’t create the Issue without them.
If the type has no assignee or location set, you can define them yourself when creating the Issue.
If you don’t choose a type, the Issue is set as “Other” and no predefined configuration applies. See Configure Issue types.
📌 Link an asset
The asset selector lets you choose a single asset per Issue. You can search by name or code, and results are sorted by asset type.
If the Issue has no type, you can choose any asset you have permission for.
If the Issue has a type, only the assets configured for that type are shown.
📌 Create an Issue offline
In the app, you can create an Issue without an internet connection, including its custom fields. The Issue syncs when the connection is back, and from then on it’s visible on the web and to other users.
📌 Frequently asked questions
Can I create an Issue without an internet connection? Yes, from the app. A connection is needed for the Issue to be visible on the web and to other users.
Can I create an Issue while editing a form that was already submitted? No. It can only be created while the form is being answered.
Do I need to finish the form to create the Issue? No, but if you don’t finish it, the Issue won’t show the form it came from.
What happens if I don’t select an Issue type? It’s automatically set as “Other” and no type configuration applies.
Why can’t I change the priority? Because the Issue type has a default priority. It can only be changed in the type’s settings.
Why can’t I create the Issue? Check that all required fields (*), required custom fields, and anything the Issue type requires (evidence, location, or asset) are filled in.
Can I link more than one asset to an Issue? No. Each Issue supports a single asset.
